How the Oregon Dental Care Plan works
for Individuals and Their Families
• $48 Annual Enrollment Fee
You pay a $48 annual enrollment fee to become a member in the Oregon Dental Care Plan. Members may pay one additional $48 annual enrollment fee to cover all dependent family members under the Plan, including spouse, children and grandchildren under the age of 23.
• Pre-Existing Conditions
There are no pre-existing condition limitations or waiting periods to worry about with the Oregon Dental Care Plan.
• Your First Appointment
Each member receives their initial exam (emergency exam or comprehensive exam) and all necessary x-rays for only $29. A comprehensive exam will be necessary and we may request a copy of your prior dental records. Your dental care team will schedule any cleanings or treatments thereafter.
• 45% Preventive Care Discount
There is no limit or annual maximum to the amount of preventive care that each member may receive. This includes hygiene cleanings, X-rays, periodic exams, sealants and fluoride treatments.
• 25% Treatment Discount
There is no limit or annual maximum to the amount of treatment care that each member may receive. This includes treating all active decay or gum disease, repairing broken teeth or dentures, and all cosmetic services. Even orthodontic services are included!
The Oregon Dental Care Plan is not dental insurance. This is a preferred provided discount dental plan in which services under the plan must be delivered by general dentists at Oregon Dental Care’s facilities. Referrals to 3rd party specialists by any licensed provider at Oregon Dental Care is not covered under the Oregon Dental Care Plan. Oregon Dental Care will provide treatment in accordance with its operating policies, including our right to refuse treatment or dismiss patients based upon adverse behavior or risk. A full copy of the Oregon Dental Care Plan fee schedule is available on request. Fees under the Plan are subject to change without notice, but the discount rates off normal fees provided above will not change. Members or their dependents may not use, combine or coordinate any Oregon Dental Care Plan benefits with any other dental plan.
